Speech Language Referral Process

Step 1 - IDENTIFICATION

Teacher Referral 

SAFE Team Referral

Speech Therapist Observation

Early Intervention Transitions

Parent Request

Doctor Referral - required for voice, swallowing and feeding



Step 2 - INFORMATION GATHERING / SCREENING

Screening is based on the specific area of concern and an informal assessment

 

Step 3 - REQUEST FOR PERMISSION TO EVALUATE


During the informal assessment if a child displays the need for formalized testing we will request parents permission to complete more formalized testing

Parent signature on the Initial Permission to Evaluate form is required

 

Step 4 - DIAGNOSTIC TESTING


Appropriate speech and/or language tests will be administered

 

Step 5 - EVALUATION REPORT


Comprehensive written report with input from parents, educators, and evaluation results will be provided to the child's team (including parents, teachers, counselors, LEA) 

If a child does not qualify for Speech and Language Support the process ends.
 A Notice of Recommended Educational Placement (NOREP) will be issued stating that the child is not eligible at this time.

 

Step 6 - INDIVIDUALIZED EDUCATION PROGRAM (IEP)

A written plan will be developed to address individual student needs.
This written plan will be reviewed and updated yearly.

 

Step 7 - NOTICE OF RECOMMENDED EDUCATIONAL PLACEMENT (NOREP)

Parent signature is required for child to receive Speech Language Support services

 

Step 8 - PLACEMENT IN SPEECH LANGUAGE SUPPORT PROGRAM

The student will begin to receive Speech and Language Support services
